APPLICATION FOR DOG ADOPTION CONSIDERATION

To be considered for adoption of a companion animal, applicants must be at least 18 years of age, have legal identification with your current address, be able to verify that a pet is welcome at the residence and understand that FoFAS reserves the right to deny adoption for any reason. Completion of this form does not guarantee adoption.

There are three options available to you in filling out the application.

Fill out the below online form - After filling out the form click the "Submit Application" button, it will automatically be sent to FOFAS. This is the Recommended method.
If you would rather fill out the form offline, there is a PDF and Microsoft Word files available for you to save. Once filled out it can be mailed to: FOFAS, 2700 Fairmont Drive, San Leandro, CA 94578-0247.
